St Helens & Knowsley Teaching Hospitals NHS Trust is one of the leading acute Trusts in the North West of England. We provide a full range of acute adult services to our local population of circa 350,000 and provide tertiary services across a much wider area in the North West, North Wales and Isle of Man. We are a Major Trauma Unit and the Mersey Regional Burns Unit.

An exciting new learning and development opportunity has arisen within the current Discharge Coordinator team. If you are an efficient worker with strong clinical knowledge, excellent communication skills and able to work under pressure whilst remaining focused on safe good quality care, seeking a new personal challenge we look forward to your application.

There is a newly created position within the AMU team working 37 1/2 hours per work over 5 days a week (Monday to Friday). You will need at least 2 years post registration experience and a sound clinical background and a valid NMC, Registered General Nurse, registration are essential in this role. We are seeking an enthusiastic, knowledgeable and self-motivated Band 5 with a ‘can do’ mindset and a strong interest in patient flow.

The AMU is a 44 bedded unit (across 2 wards) with another 12 trollies for the assessment of ED referred medical patients and an expanding GP assessment unit.

If successful you will be part of a fantastically motivated and forward thinking team who constantly strive to give the highest standards of care which involves our patients being discharged in a timely manner.

You will work closely with our ward co-ordinators to identify discharges and to ensure everything is in place for the patient to move safely to the transfer lounge and then home, through liaison with our partnership social workers, care homes and families.

You will represent the unit at our Trust discharge meetings where we highlight issues with discharges

The post holder should have up to date knowledge of current issues in patient flow and changes within the NHS. The role is challenging and instrumental to the day to day operations within the Trust.

This post will be subject to an Enhanced DBS (with barred list for vulnerable adult’s and children’s disclosure).
